WHAT ARE CHRONIC CONDITIONS
Chronic conditions, such as diabetes, heart disease and hypertension, develop over a long period of time, are prevalent in older adults and are often caused by unhealthy habits that increase the risk of disease. Unhealthy habits include poor eating habits, lack of exercise, excessive use of alcohol and smoking. Age, family history and environment also are risk factors for developing Chronic Conditions.
WHAT ARE THE SYMPTOMS?
Chronic Conditions, such as hypertension, can be asymptomatic for many years. You may only become aware of the condition when the damage is severe enough to cause an acute-type event like a heart attack or stroke.

WHAT ARE THE TREATMENT OPTIONS?
In most cases, Chronic Conditions cannot be cured. However, having been diagnosed with a Chronic Condition does not mean there is nothing you can do. Early stage diagnosis and modifying your behavior – such as quitting smoking, eating healthy and regular exercise – can help you slow the progression of the disease and control your condition.
